Who we are

Allevo Therapeutic Bodywork LLC ("Allevo," "we," "us") is a licensed massage therapy practice operated by Sarah Lotz, LMT, located at 137 S Cross St, Suite 1D, Oconomowoc, WI 53066. You can reach us at info@allevotherapeutics.com or (313) 425-5386.

What we collect

When you book, arrive for a session, or fill out our intake form, we collect:

• Your name, email address, phone number, and mailing address
• Date of birth
• Emergency contact information
• Health history relevant to massage therapy: current medications, allergies, injuries, medical conditions, pregnancy status, and other information you share on the intake form
• Session notes (chief complaint, treatment plan, therapist observations)
• Appointment history
• Payment information — processed by Stripe (see "Third-party services" below); we do not store your full card number

How we use it

We use your information only to:

• Schedule, confirm, remind, and follow up on your appointments
• Prepare for your session and provide continuity of care between visits
• Process payments
• Contact you about your care (e.g., a therapist follow-up question after a session)
• Comply with legal, tax, and licensing recordkeeping requirements

We do not sell your information. We do not share it with anyone outside the third-party services listed below, except when required by law.

Text messages (SMS)

How you opt in

You opt in to appointment text messages by entering your mobile phone number on the booking form at allevotherapeutics.com/book. The phone field is optional, and the following disclosure appears directly beneath it before you can submit:

By providing your mobile number, you agree to receive text messages from Allevo Therapeutic Bodywork about this appointment — booking confirmation, a reminder about 24 hours ahead, and any reschedule or cancellation notices. These are transactional messages only; we do not send marketing texts. Message frequency varies (typically fewer than five per booking). Msg & data rates may apply. Reply STOP at any time to opt out, or HELP for support. Consent is not a condition of booking — you can leave this field blank and we'll contact you by email, or provide a mobile number to receive SMS updates.

Providing your phone number and submitting the booking form is your affirmative opt-in. Leaving the field blank means you'll receive appointment communications by email only.

What you'll receive

• Booking confirmations, appointment reminders (about 24 hours before your session), reschedule / cancellation notices, and pre-session intake reminders when applicable.
• We do not send marketing or promotional text messages.

Message frequency and rates

• Message frequency varies — typically only when there is activity on your appointment. Most clients receive fewer than five messages per booking.
• Standard message and data rates may apply, depending on your mobile carrier plan.

How you opt out

• Reply STOP to any of our texts to unsubscribe from further SMS. You will still receive email confirmations and reminders unless you also unsubscribe from those.
• Reply HELP for support information, or email info@allevotherapeutics.com.

Sharing

We do not share your mobile phone number or your SMS opt-in status with third parties or affiliates for their marketing purposes.

Email newsletters

Separately from appointment communications, you may choose to receive occasional email newsletters and announcements from Allevo. Newsletter signup is opt-in, and every newsletter includes a one-click unsubscribe link. Unsubscribing from newsletters does not affect your transactional appointment emails.

Third-party services

We use the following third-party providers to run the practice:

• Stripe — payment processing. See Stripe's privacy policy.
• Twilio — SMS delivery. See Twilio's privacy policy.
• SendGrid (Twilio) — email delivery. See SendGrid's privacy policy.
• Hetzner — website and database hosting (Germany-based; data stored in the EU under GDPR-compliant terms).
• Cloudflare — DNS and edge networking.

Each provider receives only the data required to perform their specific function (e.g., Stripe sees payment amounts and card details but not your session notes; Twilio sees only the text message and destination phone number).

How long we keep it

We keep your health information and session records for as long as required by Wisconsin state law for massage therapy providers (currently a minimum of seven years from the date of last service). After that period, records may be securely deleted or retained if you remain an active client. Appointment history and payment records are kept for standard business and tax purposes.

Your rights

You can at any time:

• Request a copy of the information we hold about you
• Ask us to correct anything inaccurate
• Ask us to delete your information (subject to the recordkeeping requirements above)
• Withdraw consent for SMS (reply STOP) or newsletters (click unsubscribe)

Just email info@allevotherapeutics.com and we'll take care of it within a reasonable time.

Health information

Massage therapy in Wisconsin is generally not a HIPAA-covered service, but we treat the health information you share with us with the same care and confidentiality that a HIPAA-covered provider would. Session notes and intake responses are never disclosed to anyone outside the practice without your written consent, except when compelled by law.

Changes to this policy

If we update this policy, the updated version will be posted here with a new effective date. Material changes (like a new class of data collection or a new third-party processor) will also be announced by email if you're on our newsletter list.
